the wrong size bolts were in place, and it caused the windows to rattle causing the riveting around the windows to tear off.

lol airlines tell you over and over to keep ur seatbelt fastened while seated even while the seatbelt sign is not on yet the pilot just unstraps his for no reason at all. lucky he made it out alive
